Dear Staff: We want to thank each and every one of you for the excellent care you gave our mother, Marion, during her stay there. She was happy there and she made so many new friends. Your kindness, compassion and love showed each and every day. She loved her “Bingo” and sometimes Jack had to tell her to “MYOB”. She tended to worry about everyone, and God forbid if we sat where “Mary” sits, we better get out of there before Mary got back. You made her last days and months that she was with us so comfortable. Home Health Speech and Language Pathologist Per Diem

Primary Responsibilities:
• Demonstrate ability to travel to/from patient’s homes, maintaining timely and accurate reports and records.
• Demonstrates respect for patient confidentiality, goals, property and lifestyle.
• Demonstrate competence in thorough evaluation of patient’s communication, cognitive and swallowing abilities.  Sets objective goals in conjunction with patients and caregiver.
• Implement/revise appropriate treatment plans according to patient diagnosis/response/status under and in accordance with the American Speech/Language Association Code of Ethics and agency/insurance regulations.
• Demonstrate professional judgement, creativity, and a transdisciplinary approach to treatment strategies, and use of communication devices in the home.
• Coordinate a treatment and/or supervisory caseload utilizing grouping of visits per locale, time prioritization, and demonstrating productivity.
• Assist with Rehabilitation Department coverage in the absence of the Administrator as delegated.
• Complete documentation (initial evaluation, 485’s, OASIS tool, daily notes, telephone orders, conference and discharge summaries, etc.) in a timely basis utilizing Care Anyware system.
• Delegate to/supervise students and supportive personnel as appropriate.
• Ensure safety, infection control, and maintenance of any communicative devices/equipment in office area and in the patients’ home.
• Actively participate in physician and team communication and complete referrals (modified barium swallows, dsyphagia evaluations, audiology consults, etc.) to ensure quality of care and appropriate discharge plans.
• Provide education to patient/caregiver during treatment to ensure appropriate carryover.
• Enhance clinical skills through participation in inservice, continuing education, research review of current literature as appropriate.  Participate in department/hospital/community educational activities.
• Assist with Performance Improvement activities within the departments/hospital to improve patient/staff and quality of patient care, etc.
• Maintain contact with Whittier Rehabilitation Hospital’s Communication Disorder Department as needed for clinical support.

Prerequisites:
• Master’s of Science degree in Speech/Language Pathology from an ASHA accredited program.
• Certificate of clinical competence in Speech/Language Pathology by the American Speech/Language/Hearing Association.
• Current licensure in Massachusetts and/or New Hampshire.
• Knowledge of growth and development across the life span.
• Certification in BCLS of Adult.
• Must have valid driver’s license and access to reliable transportation.
• National and State Speech/Language/Hearing Association membership preferred.
• Completion of hospital and agency orientation and reorientation as required.
• Two years of experience preferable; in acute care, rehabilitation or home care including age-related competence for pediatric, adolescent, adult, and geriatric patients as indicated. 
• Minimum requirement: 1.) Meets the education and experience requirement for a Certificate of Clinical Competence in speech pathology or audiology granted by The American Speech-Language-Hearing Association: or 2.) Meets the education requirements for certification and is in the process of accumulating the supervised experience required for certification.
