Thank You to the Staff at Oak Knoll Healthcare Center Thank you for your care and kindness while I was there to begin recuperating from a stroke I had on October 11, 2016...Whatever your role, thank you for your patience when I had a bad day...your strength when I didn’t have any, your encouragement when I had doubts, your “pep talks” when I was discouraged and needed them most, your sense of humor to add some laughs to the day (laughter is the best and cheapest medicine). Some of these may seem like little things to you, but they make the patient feel more like a person who matters instead of just the occupant of a specific bed. Home Health Physical Therapist - $5000 Sign On Bonus for Full Time
Full Time 40 hours / to work in our Westboro branch
Per Diem also available

Primary Responsibilities:
• Provide individualized evaluation, comprehensive planning, development of goals, implementation of treatment and discharge plans, documentation of findings and progress, and patient/family education in the home setting under the direction of a licensed physician.

Prerequisites:
• Bachelor of Science/Master’s of Science degree in physical therapy, DPT, from an APTA accredited college or university.
• Current licensure by the Board of Allied Health Professionals, Commonwealth of Massachusetts or application for the same. 
• APTA in Massachusetts and/or New Hampshire chapter of APTA membership preferred.
• Certification in BLS of Adult, Child and Infant.
• Demonstrated pursuit on continuing education courses relative to the field of physical therapy.
• Knowledge, experience, and competency in dealing with adult rehabilitation and geriatric age group. 
• Knowledge of growth and development across the life span.
