Dear Doc and the Boys, On behalf of the entire family I want to thank you for the absolutely outstanding care that was given to our mother at the Masco Health Center. The care that was given to her was like having the same care that would be given by a family member. The facility is beautiful, extremely clean and the staff was magnificent. Renee the activities director along with Lorrie Nobil, Cheryl Clapp and the rest of the staff made her final days peaceful and serene. Thank you all so much for your dedication and love. Social Worker for TCU

Location: Westborough, MA - Whittier Westborough Transitional Care Unit

Social Worker for TCU – Part Time 24 hours per week
This is an exciting opportunity to be part of the team for the 19 bed TCU located within Whittier Rehabilitation Hospital at Westborough!


Primary Responsibilities:
• May participate on the Admission Team, with the Administrator and Director of Nursing, to assess the appropriateness of referrals for admission to the facility.  This is done prior to the admission of the resident.
• Participates in the formation of the total care plan for each resident based on medical, nursing and psycho-social needs of the resident.  Resident and family are involved to the extent feasible in formulating the care plan.
• May document pre-admission activities in the clinical record or each new admission (i.e., pre-admission facility tours, interview, etc.)
• Shortly after admission, develops the social service evaluation for the clinical record, to include the resident's social history, social and emotional need of the resident, working goals and treatment plan.
• Assists resident with adjustment to nursing home at the time of admission and throughout placement.
• Liaisons with family members and responsible parties.
• Serves as an advocate for Residents' Bill of Rights.
• Attends resident's councils.
• Participates in inservice education programs within nursing home and outside facilities to maintain CEU credits for licensure.
• Provides ongoing inservices regarding Residents’ Bill of Rights, Patient Abuse Statute and Death and Dying, etc.
• Coordinates with outside agencies, i.e., mental health organizations, to meet psycho-social and emotional needs of residents.
• Implements working knowledge of Medicare/Medicaid bylaws.
• Participates in Continuous Quality Improvement.
• Attends weekly patient care planning meetings.
• Attends weekly Medicare meetings.
• Participates in preparation for JCAHO.
• Maintains an average of fifty (50) resident case load.
• Prepares quarterly progress notes.
• Conducts Crisis Intervention with facility personnel and residents.
• Develops and implements discharge plan in coordination with resident, family and facility team.
• Provides discharge/expiration follow-up.

Prerequisites:
• Must be licensed by the Commonwealth of Massachusetts.
• Must have at least a BA in Social Work or other related discipline from an accredited university.
• At least two (2) years work experience as a social worker required with experience in a health care setting preferred.
• Possess working knowledge of state and federal programs and regulations affecting nursing homes preferred.  Possess general knowledge of community health and social resources preferred.
• Knowledge of growth and development across the life span required.
