On May 25, 2014, my wife, Janice H., passed away at the age of 49, due to complications from a 28 year battle with multiple sclerosis. She had been under the great care of Oak Knoll Healthcare Center for over eleven years. She was treated with the utmost respect and dignity that could ever be expected. Some of the staff went above and beyond to make her life as comfortable as possible. I have to write you this letter to tell you how much my family and myself appreciate what Oak Knoll did for Janice in helping her ”live” at Oak Knoll for eleven years. Location: Westborough, MA - Whittier Westborough Transitional Care Unit

MDS Nurse (RN / LPN - TCU) Full Time 40 hours Monday through Friday

Primary Responsibilities:
• Ensures accurate and timely completion of all resident assessment instruments including MDS sheets, face sheets, trigger sheets, RAP summaries, and care plans.
• Updates staff on new and/or revised laws and interpretations of laws governing resident assessment.
• Upon a resident's admission, assists in initiating the assessment process.
• Completes nursing sections of the MDS and/or 90 day (quarterly) MDS for all medicare/managed care patients.
• During the comprehensive assessment following admission, circles/highlights appropriate triggers and pulls corresponding RAPs for completion.
• Completes nursing RAPs as needed.
• Assists with other MDS's as needed when medicare/managed care census is low.
• Signs the back of each nursing trigger sheet once each trigger has been appropriately addressed, a statement made on each regarding to location of information, and date has been addressed.
• Assumes the same responsibilities with all Minimum Data Sets started secondary to patient change in status and all significant corrections.
• May electronically transmit per MDS assessment schedule, but at least monthly.
• Tracks consistency between documentation.

• Graduate from an accredited LPN or Registered Nursing Associate's Degree Program required.
• Current License to practice as an LPN or Registered Nurse in the Commonwealth of Massachusetts.
• Two years previous experience in long term care.  
• Knowledge of DPH, OBRA, JCAHO, Medicare regs and MDS process.
• Experience working within an interdisciplinary team approach to care.
• Computer skills.
• Knowledge of growth and development across the life span required.
