Activity Director

FT Monday-Friday - no weekends

Responsibilities:
- Plans, schedules and implements activity program of individual and group activities based on residents’ needs, abilities and interests.
- Completes the activity component of the resident assessment within the time period required by state regulations for new admissions annually.
- Identifies and documents in the resident’s care plan the activity interests, needs and problems of the resident. Care planning updates will be documented every 3 months or as condition changes.
- Acts as an active member of the multi-disciplinary team and all related resident care conferences.
- Plans and implements Reality Orientation and Validation on a one-to-one basis.
- Plans and conducts Residents’ Council monthly.
- Takes part in in-service education and attends all in-service sessions relating to the Activity Program and the facility.
- Plans, schedules and implements room visits and in-room activities for residents unable to leave their rooms.
- Maintains an adequate supply of materials to implement activity programs.
- Observes infection control procedures related to the Activity Program.
- Responsible for the safety of residents under his/her supervision.

Prerequisites:
- High School diploma and completion of the Activity Director’s Basic Course required and, at least one year experience directing an Elderly Recreational Program;
or
- At least 2 years experience as an Activity Assistant;
or
- Bachelor’s degree in recreational therapy or related field and prior experience with the elderly.


Weekend Baylor Nurse
R.N. for every other weekend 7:00am to 7:00pm

Prerequisites:
- MA Nursing License
- Long Term Care experience preferred
