Summary: Patients at WRH-W had a higher LOS efficiency than the nation, meaning they made a higher gain in function per day while at WRH-W.  The Discharge FIM and the FIM Change of WRH were lower in scores than the Nation.  WRH-W had a higher Cognition Score Change and lower Motor Score Change when compared to the nation.  WRH-W discharged a lower percentage of patients to the community than the nation.  WRH-W discharged a higher percentage of Acute Discharges than the nation.  WRH-W discharged a higher percentage of patients to LTCF than the nation.  We had an increase in D/C FIM from Q2  to Q3 from 80.6 to 85.6.  There was an increase in FIM Change from Q2 to Q3 from 17.7 to 20.2.  We had an increase in LOS Efficiency from Q2 to Q3 from 1.22 to 1.31.  Motor Score Change from Q2 to Q3 increased from 17.3 to 19.0.  Change in cognition increased from Q2 to Q3 from 0.4 to 1.2.  Our % of discharges from Q2 to Q3 increased for Community from 49% to 52% and Acute decreased from 23% to 22%.  LTCF discharges decreased from Q2 at 28% to Q3 at 26%.  Mortality rate decreased from Q2 to Q3 from 1.1%  to 0%.

Summary: Patients at WRH-W had a higher LOS efficiency than  the nation, meaning they made a higher gain in function per day while at WRH-W.  The Discharge FIM and the FIM Change of WRH were lower in scores than the Nation.  WRH-W had a lower Cognition Score Change and Motor Score Change when compared to the nation.  WRH-W discharged a lower percentage of patients to the community than the nation.  WRH-W discharged a higher percentage of Acute Discharges than the nation.  WRH-W discharged a higher percentage of patients to LTCF than the nation.  We had an increase in D/C FIM from Q1 to Q2 from 79.6 to 80.6.  There was a decrease in FIM Change from Q1 to Q2 from 19.0 to 17.7.  We had an increase in LOS Efficiency from Q1 to Q2 from 1.15 to 1.22.  Motor Score Change from Q1 to Q2 increased from 17.2 to 17.3.  Change in cognition decreased from Q1 to Q2 from  1.8 to 0.4.  Our % of discharges from Q1 to Q2 increased for Community from 44% to 66% and Acute decreased from 26% to 23%.  LTCF discharges increased from Q1 at 29% to Q2 at 28%.  Mortality rate increased from Q1 to Q2 from 0.6%  to 1.1%.

Summary: Patients at WRH-W had a higher LOS efficiency than the nation, meaning they made a higher gain in function per day while at WRH-W.  The Discharge FIM and the overall FIM Change of WRH were lower in scores than the Nation.  WRH-W had a higher cognition score than the Nation.  WRH-W had a lower Motor Score Change when compared to the Nation.  WRH-W discharged a lower percentage of patients to the community that the nation.  WRH-W discharged a higher percentage of Acute Discharges than the nation.  WRH-W discharged a higher percentage of patients to LTCF than the nation.  We had a decrease in D/C FIM from Q4 to Q1 from 82.4 to 79.6.  There was an increase in FIM Change from Q4 to Q1 from 16.0 to 19.0.  LOS Efficiency from Q4 to Q1 stayed the same at 1.15.  Motor Score Change from Q4 to Q1 increased from 16.2 to 17.2.  Change in cognition increased from Q4 to Q1 from 0.6 to 1.8.  Our % of discharges from Q4 to Q1 decreased for Community from 66% to 44% and Acute increased from 17% to 26%.  LTCF discharges increased from Q4 at 18% to Q1 at 29%.  Mortality rate decreased from Q4 to Q1 from 1.1% to 0.6%.
