Activity Assistant 24 hours/week, including Saturdays
Responsibilities:
- Assists with the completion of the activity component of the resident assessment within the time period required by state regulations for new admissions annually - Assists with identifying and documenting in the resident's care plan the activity interests, needs and problems of the resident. Care planning updates will be documented every 3 months or as conditions change. - Consistently works cooperatively and maintains effective communication with facility administration, nursing personnel, all facility departments and consultants. - Maintains a daily activity attendance record for each resident. - Observes infection control procedures related to the Activity Program. - Responsible for the safety of residents under his/her supervision. - Leads activity progran on weekdays, as well as evenings, weekends, and holidays, per resident needs and program requirements. - Supports, encourages and motivates independent activity and pursuits, both individually and in small groups.
Prerequisites:
- High School diploma or G.E.D. plus, at least 2 years experience working with the elderly OR - At least 1 year experience in Elderly Recreation Program OR - Successful completion of the Activities Director Basic Course.
CNA Full-time, 11pm-7am; Per Diem
Responsibilities:
- Assist patients with activities of daily living specific to culture, age and developmental level - Performs tasks/tests delegated by the registered nurse and reports/records data as needed. - Performs advanced skills delegated by the registered nurse as deemed competent, reports/records data as needed. - Continuously monitors/observes patients to assure needs are met in a timely manner. - Reports any changes observed in condition or behavior of patients and unusual incidents.
