MDS Coordinator Full Time 40 hours
Responsibilities: - Ensure accurate and timely completion of all resident assessment instruments including MDS sheets, face sheets, trigger sheets, RAP summaries and care plans. - Update staff on new and/or revised laws and interpretations for laws governing resident assessment. - Assure that the MDS is appropriately completed and signed by each discipline before the end of each patient's assessment schedule. - Oversee care planning from day 14 through completion on day 21, assisting nursing on an as-needed basis.
Prerequisites: - Registered Nurse licensed by the Commonwealth of Massachusetts. - Two years previous experience in long term care. - Knowledge of DPH, OBRA, JCAHO, Medicare regs and MDS process. - Experience working within an interdisciplinary team approach to care. - Computer skills.
Staff Nurse Full Time 11p-7a
Responsibilities: - Supervises and evaluates all direct resident care and initiates corrective action as necessary. - Orders, receives, documents medication received according to facility procedure. - Performs comprehensive assessment of each resident. - Provides direct resident care and gives instructions to nursing assistants related to resident care. - Administers and documents medications, enteral nutrition, and treatment per the physicain's order and accurately records all care provided and observations daily.
Prerequisites: - Graduate of an accredited RN or LPN school of nursing. - Current license in Massachusetts as RN or LPN. - Knowledge in Geriatric Nursing required. - Current certification in Basic Cardiac Life Support in accordance with American Heart Association or American Red Cross standard required.
Nursing Scheduler Full Time Mon-Fri
Responsibilities: - Maintain filing and records management systems. - Assist in weekly payroll activities by preparing time cards for submission to bookkeeper. - Assist in the scheduling of nursing employees by helping to initiate and maintain monthly schedule, prepare daily daily staffing sheets, and tracking employee attendance.
Prerequisites: - High School graduate, some business schooling or course preferred. - Ability to read, write, speak, and comprehend the English language with good communication skills. - Maturity and ability to work with little supervision and to maintain necessary level of confidentiality. - Geniune desire to work with the elderly in a health care setting.
Charge Nurse Full Time Mon-Fri also on-call rotation
Responsibilities: - Supervises and evaluates all direct resident care and initiates corrective action as necessary. - Provides clinical supervision of nursing assistants providing direct patient care. - Prepares nusing assistants' assignments and explains all pertinent resident care. - Orders, receives and documents medication received according to facility procedure.
Prerequisites: - Graduate of an accredited school of nursing. - Current license in Massachusetts as RN or LPN. - Knowledge of Geriatric Nursing required. - Current certification in Basic Cardiac Life Support in accordance with the American Heart Association or American Red Cross Standards required.
Director of Nursing Full Time
Responsibilities: - Assumes responsibility to lead and manage the Patient Care Staff. - Consistently operates nursing service within budget guidelines established. - Plans and facilitates staff meetings with all shifts for purposes of planning, coordination and implementing nursing service policies and procedures. - Directs, evaluates and supervises all resident care and initiates corrective action as necessary. - Handles nursing service problems, emergency situations, and initiates "life-saving" measures within the scope of nursing practice. - Also takes responsibility in the absence of the Administrator.
Prerequisites - Must be a graduate of an accredited school of nursing, BS preferred. - Current licensure in Massachusetts as a Registered Nurse. - Minimum of 5 years recent clinical long term care experience. - Minimum of 3 years supervisory experience in a long term care facility. - Knowledge of managed care, geriatric care, reimbursement systems, OBRA, HCFA, JCAHO and Medicare required. - Knowledge in areas of management, labor relations, recruitment, budgeting and finance. - Current Certification in Basic Cardiac Life Support. - Knowledge of growth and development across the life span required.

Prerequisites: - Must be ServSafe certified. - Experience preferred.
Speech Therapist Per Diem
Responsibilities: - Work as part of an interdisciplinary team with our outpatient and/or inpatient populations. Experience with general acute rehabilitation and Videofluoroscopy studies a plus.
Prerequisites: - Masters in communication disorders from ASHA-accredited program - Current Licensure by the Board of Allied Health Professionals, Commonwealth of Massachusetts - Current CPR certification
PTA Per Diem weekends
Responsibilities: - Provide individualized evaluation, documentation of findings and progress, comprehensive planning and implementation of treatment plan, patient/family education and determination of rehabilitation and patient goals under the direction of a physician.
Prerequisites: - Associates degree in physical therapy from an APTA-accredited program. - Current licensure by the Board of Allied Health Professionals, Commonwealth of Massachusetts or application for the same. - ATPA and Massachusetts or New Hampshire Chapter of ATPA Membership proffered. - Certification in BLS of Adult, child and Infant. - Demonstrated pursuit of continuing education courses relevant to the field of physical therapy. - Demonstrated ability with interpersonal and organizational skills. - Knowledge, experience and competency in dealing with adult rehabilitation and geriatric age group. - Knowledge of growth and development across the life span.
OTR/L Per Diem
Responsibilities: - Assess and implement appropriate treatment plans according to patient diagnosis and status and in accordance of the Occupational Therapy Code of Ethics. - Completes documentation (e.g., initial evaluation, daily flow sheets, weekly progress notes, discharge summaries and statistics) on a timely basis. - Completes letters of justification for Durable Medical Equipment. - Delegate appropriate duties to certified occupational therapy assistants, aides and supportive personnel. - Coordinate and organize a full treatment case load. - Attends and reports at rehabilitation team conferences (RTC), treating team conferences (TTC) and family team conferences (FTC). - Seek to provide or enhance staff, patient and family education through continuing education courses/seminars and review of current relevant literature.
Physical Therapist Per Diem (Every weekend)

CNA’s All Shifts
Responsibilities: -Assist patients with activities of daily living specific to culture, age and developmental level -Performs tasks/tests delegated by the registered nurse and reports/records data as needed. -Performs advanced skills delegated by the registered nurse as deemed competent, reports/records data as needed. -Continuously monitors/observes patients to assure needs are met in a timely manner. -Reports any changes observed in condition or behavior of patients and unusual incidents.
