Director of Nursing
Full Time

Responsibilities:
- Assumes responsibility to lead and manage the Patient Care Staff.
- Consistently operates nursing service within budget guidelines established.
- Plans and facilitates staff meetings with all shifts for purposes of planning, coordination and implementing nursing service policies and procedures.
- Directs, evaluates and supervises all resident care and initiates corrective action as necessary.
- Handles nursing service problems, emergency situations, and initiates "life-saving" measures within the scope of nursing practice.
- Also takes responsibility in the absence of the Administrator.

Prerequisites
- Must be a graduate of an accredited school of nursing, BS preferred.
- Current licensure in Massachusetts as a Registered Nurse.
- Minimum of 5 years recent clinical long term care experience.
- Minimum of 3 years supervisory experience in a long term care facility.
- Knowledge of managed care, geriatric care, reimbursement systems, OBRA, HCFA, JCAHO and Medicare required.
- Knowledge in areas of management, labor relations, recruitment, budgeting and finance.
- Current Certification in Basic Cardiac Life Support.
- Knowledge of growth and development across the life span required.


Cook

Prerequisites:
- Must be ServSafe certified.
- Experience preferred.


Speech Therapist

Per Diem

Responsibilities:
- Work as part of an interdisciplinary team with our outpatient and/or inpatient populations. Experience with general acute rehabilitation and Videofluoroscopy studies a plus.

Prerequisites:
- Masters in communication disorders from ASHA-accredited program
- Current Licensure by the Board of Allied Health Professionals, Commonwealth of Massachusetts
- Current CPR certification


PTA
Per Diem weekends

Responsibilities:
- Provide individualized evaluation, documentation of findings and progress, comprehensive planning and implementation of treatment plan, patient/family education and determination of rehabilitation and patient goals under the direction of a physician.

Prerequisites:
- Associates degree in physical therapy from an APTA-accredited program.
- Current licensure by the Board of Allied Health Professionals, Commonwealth of Massachusetts or application for the same.
- ATPA and Massachusetts or New Hampshire Chapter of ATPA Membership proffered.
- Certification in BLS of Adult, child and Infant.
- Demonstrated pursuit of continuing education courses relevant to the field of physical therapy.
- Demonstrated ability with interpersonal and organizational skills.
- Knowledge, experience and competency in dealing with adult rehabilitation and geriatric age group.
- Knowledge of growth and development across the life span.


OTR/L
Per Diem

Responsibilities:
- Assess and implement appropriate treatment plans according to patient diagnosis and status and in accordance of the Occupational Therapy Code of Ethics.
- Completes documentation (e.g., initial evaluation, daily flow sheets, weekly progress notes, discharge summaries and statistics) on a timely basis.
- Completes letters of justification for Durable Medical Equipment.
- Delegate appropriate duties to certified occupational therapy assistants, aides and supportive personnel.
- Coordinate and organize a full treatment case load.
- Attends and reports at rehabilitation team conferences (RTC), treating team conferences (TTC) and family team conferences (FTC).
- Seek to provide or enhance staff, patient and family education through continuing education courses/seminars and review of current relevant literature.


Physical Therapist
Per Diem (Every weekend)

Responsibilities:

Prerequisites:
- Bachelor of Science/Masters of Science degree in physical therapy from an APTA accredited college or university.
- Current licensure by the Board of Allied Health Professionals, Commonwealth of Massachusetts or application for the same.
- APTA in Massachusetts and/or New Hampshire Chapter of APTA, membership preferred.
- Certification in BLS of Adult, Child and Infant.
- Demonstrated pursuit on continuing education courses relative to the field of physical therapy.
- Knowledge, experience and competency in dealing with adult rehabilitation and geriatric age group.
- Knowledge of growth and development across the life span.


COTA

Per Diem (Every weekend)

Prerequisites:
-Associates degree in occupational therapy from an AOTA-accredited program.
-Current licensure by the Board of Allied Health Professionals, Commonwealth of Massachusetts.
-American Occupational Therapy Association membership preferred.
-Certification in BLS of Adult, Child and Infant.
-Demonstrated pursuit of continuing education courses relevant to the field of occupational therapy.
-Demonstrated ability with interpersonal and organizational skills.
-Knowledge, experience and competency in dealing with adult rehabilitation and geriatric age group.
-Knowledge of growth and development across the life span.



CNA’s
All Shifts

Responsibilities:
-Assist patients with activities of daily living specific to culture, age and developmental level
-Performs tasks/tests delegated by the registered nurse and reports/records data as needed.
-Performs advanced skills delegated by the registered nurse as deemed competent, reports/records data as needed.
-Continuously monitors/observes patients to assure needs are met in a timely manner.
-Reports any changes observed in condition or behavior of patients and unusual incidents.
