Home Health RN

Full Time (Psych Nursing Experience Preferred)
Per Diem

Responsibilities:
Provide comprehensive nursing care to clients based on assessment of their needs and results of on-going evaluation within the framework of Community Health Nursing Practice.

1. Formulates and implements a home health care plan for the client which is based on physician orders, the therapeutic plan of other members of the health care team and nursing assessment. Considers all factors which affect client health status, e.g., lifestyle alterations, self-care deficits, critical events, pathophysiology and support network.
2. Assumes accountability and responsibility for a client caseload, within the accepted standards of Community Health Nursing practice.
3. Provides those services requiring substantial and specialized nursing skill.
4. Regularly reevaluates the client’s nursing needs and makes necessary revisions to the plan of care.
5. Initiates appropriate preventive and rehabilitative nursing procedures.

Prerequisites:
1. Graduate of an approved school of professional nursing.
2. Licensed to practice as a Registered Nurse by the State of Massachusetts and/or New Hampshire.
3. One year of professional experience preferable; in acute care, rehabilitation or home care including age-related competence for adult, and geriatric patients as indicated.
4. Knowledge of growth and development across the life span.
5. Certification in BCLS of Adult.


Home Health Physical Therapists
Full Time
Per Diem

Responsibilities:
Provide individualized evaluation, comprehensive planning, development of goals, implementation of treatment and discharge plans, documentation of findings and progress, and patient/family education in the home setting under the direction of a licensed physician.

1. Demonstrate ability to travel to/from patient’s homes, maintaining timely and accurate reports and records.
2. Demonstrates respect for patient confidentiality, goals, property and lifestyle.
3. Demonstrate competence in thorough evaluation of patient’s functional performance and home safety/accessibility. Set measurable and objective goals in conjunction with patient/caregiver.
4. Implement/revise appropriate treatment plans according to patient diagnosis/response/status under and in accordance with the Physical Therapy Code of Ethics and agency/insurance regulations.
5. Demonstrate professional judgment, creativity, and a trans-disciplinary approach to treatment strategies, and use of adaptive equipment in the home.
6. Coordinate a treatment and/or supervisory caseload utilizing grouping of visits per locale, time prioritization, and demonstrating productivity.

Prerequisites:
1. Bachelor of Science degree or Master of Science degree in Physical Therapy from an APTA accredited program.
2. Current licensure in Massachusetts and/or New Hampshire.
3. Knowledge of growth and development across the life span.
4. Certification in BCLS of Adult.
5. Must have valid driver’s license and access to reliable transportation.
National and State Physical Therapy Association membership preferred.


Home Health Social Worker
Per Diem (NH License required)

Responsibilities:
- To provide social work intervention for clients and/or families that have demonstrated a need for such while receiving home health care services; to provide necessary support to team members involved in client care.
- Demonstrates ability to travel to/from patient’s homes, maintaining timely and accurate reports and records.
- Evaluates the client’s psychological status as it relates to the client’s illness and environment.
- Assists the physician and other team members in understanding the significant social and emotional factors related to the health problems of individual clients.
- Prepares clinical and progress notes in timely, accurate manner.
- Participates in case conferences with the health care team.
- Works with the family and/or significant others.
- Utilizes appropriate community resources.
- Participates in discharge planning and inservice programs.
- Acts as a consultant to other department personnel.
- Identifies client and family needs for other home health services and refers for same when appropriate.
- Delegates to/supervises social worker assistants.
- Coordinates a treatment and/or supervisory caseload utilizing grouping of visits per locale, time prioritization, and demonstrating productivity.
- Ensures safety, infection control, and maintenance of equipment in office area and in the patients’ home.
- Participates in the agency’s planning for social service programs and policy formulation.
- Assists with Performance Improvement activities within the departments/hospital to improve patient/staff and quality of patient care, etc.

Prerequisites:
- Master’s degree from a school of social work accredited by the council on social work education.
- One year of social work experience in a health care setting including age-related competence for pediatric, adolescent, adult, and geriatric patients as indicated.
- Current licensure in Massachusetts and/or New Hampshire.
- Knowledge of growth and development across the life span.
- Certification in BCLS of Adult.
- Must have valid driver’s license and access to reliable transportation.
- Working knowledge of therapeutic modalities and community resources.
- Must demonstrate good communications, verbal and written skills.


Home Health LPTA
FT (with sign-on bonus)

Prerequisites:
-Associates degree in physical therapy from an APTA accredited program.
-Current Licensure by the Board of Allied Health Professionals, Commonwealth of Massachusetts or application for the same.
-APTA and MA or NH Chapter of APTA Membership offered.
-Certification in BLS of Adult, Child and Infant.
-Demonstrated pursuit of continuing education courses relevant to the field of Physical Therapy.
-Demostrated ability with interpersonal and organizational skills.
-Knowledge, experience and competency in dealing with adult rehabilitation and geriatric age group.
-Knowledge of growth and development across the life span.
