Dear Ms. Grasso: Our entire family would like to sincerely thank your facility and its staff for the marvelous job they all did in caring for Teddy L. Without exception each one of your staff gave sincere, loving, compassionate, empathetic, nurturing and patient care as we all worked through this climactic event. You should be very proud to have a team of such excellent professionals. Tun, Jeff, Beth, Ann, Anna, Brian, were just some of your team members that were interfacing with us so delicately. I believe Tun even stayed several evenings well past his shift to sit with Teddy - a sacrifice of love. When I told my friends about the excellent care you provided by repositioning, cleaning and changing Teddy so frequently, they were so impressed. It kept our loved one very calm and comfortable. The entire process was like a feather floating to the floor. Your professionals made the best out of an awful reality. Please forgive me by not mentioning by name all the ones that gave such incredible support. We look forward to working with you all as Peggy is still in your capable hands. Social Worker - Per Diem

Social Worker Per Diem

Primary Responsibilities:
• May participate on the Admission Team, with the Administrator and Director of Nursing, to assess the appropriateness of referrals for admission to the facility. This is done prior to the admission of the resident.
• Participates in the formation of the total care plan for each resident based on medical, nursing, and psycho-social needs of the resident.
• Resident and family are involved to the extent feasible in formulating the care plan.
• May document pre-admission activities in the clinical record or each new admission.
• Shortly after admission, develops the social service evaluation for the clinical record, to include the resident's social history, social and emotional need of the resident, working goals and treatment plan.

Prerequisites:
• Must be licensed by the Commonwealth of Massachusetts.
• Must have at least a BA in Social Work or other related discipline from an accredited university.
• At least two (2) years work experience as a social worker required with experience in a health care setting preferred.
• Possess working knowledge of state and federal programs and regulations affecting nursing homes preferred.
• Possess general knowledge of community health and social resources preferred.
• Knowledge of growth and development across the life span required.
