Home Health Physical Therapy Assistant – Per Diem

Primary Responsibilities:
• Implements individualized treatment programs and discharge plans per plan of care, document findings and progress, provide patient/family education in the home settings under the direction of a licensed physical therapist and licensed physician.
• Demonstrate ability to travel to/from patient’s homes, maintaining timely and accurate reports and records.
• Demonstrate respect for patient confidentiality, goals, property, and lifestyle.
• Function independently in the implementation and progressing treatment plan according patient diagnosis/response/status under the direction of a physical therapist in accordance with the principles of Physical Therapist Assistant Code of Ethics and agency/insurance regulations.
• Implement specific interventions, treatments and/or care required by the adolescent through geriatric age groups.
• Assess learning needs of patient and family considering age, and developmental level, as well as cultural and religious practices, language and emotional barriers, and readiness to learn.
• Teach and plan specific measures to meet the needs as appropriate to the age and developmental level of the patient and family.
• Communicate and present information clearly and in a manner to the age and developmental level of the patient/family.
• Demonstrate professional judgment, creativity, and a transdisciplinary approach to treatment strategies, and use of adaptive equipment in the home.
• Coordinate a treatment caseload utilizing grouping of visits per locale, time prioritization, and demonstrating productivity.
• Complete documentation (daily notes,billing,rehabilitation team conference sheets, obtaining co-signatures as indicated on change in service and notification of termination of service forms.)
utilizing Care Anyware system.
• Ensure safety, infection control, and maintenance of equipment in office area and in the patients’ homes.
• Actively participate in team communication to ensure quality of care and appropriate discharge plan.
• Provide education to patient/caregiver during treatment to ensure appropriate carryover.
• Enhance clinical skills through participation in in-service, continuing education, research review of current literature as appropriate.
• Assist with Performance Improvement activities within the departments/hospital to improve patient/staff and quality of patient care, etc.
• Delegate to students and supportive personnel as appropriate.

• Associates degree in Physical Therapy from an APTA accredited program.
• Current licensure in Massachusetts and/or New Hampshire.
• Knowledge of growth and development across the life span.
• Certification in BCLS of Adult.
• Must have a valid driver’s license and access to reliable transportation.
• National and State PT Association membership preferred.
• Completion of hospital and agency orientation and reorientation as required.
• Two years of current therapy experience; preferably in acute care, rehabilitation or home care including age-related competence for pediatric, adolescent, adult, and geriatric patients as indicated.
