We are writing to express our sincere gratitude to the members of your staff who care for our mother, Lucille C. The first person we want to acknowledge is Linda Tuxbury. Linda has been our support from day one at Port Rehab. She handles all the correspondence that we received through the Mass Health application process. We trust Linda completely and feel that she goes over and beyond her duties. We are happy with Lucille’s care on the floor as well, she is always well kept and seems very happy. Port Healthcare provides us with the peace of mind knowing that our mom is being well taken care of. Lastly, we want to thank Joe for being our first point of contact at Port. Home Health Intake Coordinator

Location: Bradford, MA - Whittier Home Health Care Agency

Home Health Intake Coordinator Full Time 40 hours/week Monday-Friday

Primary Responsibilities:
• Facilitates and coordinates the intake referral admission process for Agency.
• Consults with physicians, nurses, discharge planners and other health care professionals to facilitate a safe, smooth transition from the hospital to home by coordinating services and equipment needs for clients prior to their admission.
• Documents pertinent information regarding client’s demographics, medical condition, medications and treatments, and services being requested.
• Completes intake process from initial referral contact to home care admission.
• Initiates initial and ongoing commercial insurance authorization. Documents information regarding reimbursement and coordinates verification of funding sources. Understands insurance reimbursement guidelines.
• Fields all requests for private services, is knowledgeable of rates, reimbursements, contracting procedures, explaining types of services available.
• Reports problems of a clinical nature to Clinical Director/Clinical Supervisor or Administrator as necessary.
• Explains Agency’s policies, procedures and programs to referral sources, clients and families. Follows through on requests for marketing information.
• Initiates and completes discharge chart analysis.
• Coordinates with the Home Health Staffing Coordinator to assure that scheduling of visits and client visits are clinically appropriate and performed by the appropriate clinician.

Prerequisites:
• High school graduate.
• Must demonstrate an ability to work with other health care providers, and knowledge of third party reimbursement. 
• Good verbal and written communication skills required.
• Must possess good organizational and learning skills.
• Knowledge of growth and development across the lifespan.
