Dear Ms. Grasso, I just wanted to take a minute to commend you and your staff. My mother, Gilberte H., has been in and out of Port Rehab since November 2018. It does help when I am there and witness the care and compassion extended to her in her time of need. I wish I could think of the names of all the staff there who I feel have not only done their job well, but went over and above, but I will mention the few names that stand out to me. Cara has been especially patient and kind with Mom. Paul makes her smile and laugh, and the one who really prompted me to even think to write this letter is David. I believe he’s an LPN; he administers her medications. He treats her with incredible compassion, gentleness, and kindness and allows her the dignity she deserves. I don’t want to leave out all the Occupational and Physical Therapists who work with Mom. They are all exceptionally professional and wonderful. Dietitian Per Diem

Primary Responsibilities:
• Responsible for all aspects of patient's nutritional care, which includes formulating nutritional care plans and overseeing their implementation.
• Assure ADA standards for protocols are followed, and make recommendations to appropriate clinical personnel.
• Confer with Medical and Nursing Staff regarding patient’s nutritional status and attend related patient care conferences.
• Responsible for nutritional assessment, services and discharge planning; may refer patient’s family to Outpatient Services, as necessary.
• Assist in developing and implementing the Culinary & Nutritional Department CQI activities.
• Provide nutritional counseling and evaluation services to the Outpatient Cardiac Rehabilitation Clientele and to the Whittier Home Health Care Agency.
• Responsible for educational sessions regarding diets and eating habits to the Outpatient Cardiac Rehabilitation clientele and Whittier Home Health Care Agency.
• Responsible for overall supervision of Culinary Department clinical personnel; includes orientation, training and evaluating performance.
• Demonstrate geriatric and rehabilitation knowledge and skills to patient care.

Prerequisites:
• Bachelor of Science Degree in Nutritional Sciences.
• Maintain registration with the American Dietetic Association.
• At least two (2) years of related administrative and clinical experience in a health care facility.
• Geriatric and Rehabilitation experience preferred.
• Knowledge of growth and development across the life span.
