To Hannah Duston Staff: Mom was admitted to the hospital - She had a bad spell. She needed much care - she didn’t feel well. After a time, her condition changed to fair. She was almost herself - but not quite there. She couldn’t stay at the hospital, yet couldn’t be alone. What could we do until she was ready for home? We looked around almost everywhere, For a placed that offered specialized care. We searched high and we searched low. Then decided Hannah Duston - was where she would go. We were nervous when she arrived - didn’t know what to expect. She was immediately welcomed with the utmost respect. We were afraid this place would make her feel blue. We were pleased to find the opposite was true. Everyone was so friendly from each nurse to those who clean. The atmosphere was the cheeriest that I’ve ever seen. Each OT, PT, each nurse, every food service worker every CNA Receptionist, hairdresser, social worker, the administrator went out of their way, To make mom feel comfortable and welcomed from the very first day. She has improved tremendously - due to Hannah Duston’s care When mom needed you - we are glad you were there. She is headed back to her home - Many thanks to ALL of you, We are eternally grateful for all that you do. Home Health Social Worker – Per Diem

Primary Responsibilities:
• Provides social work intervention for clients and/or families that have demonstrated a need for such while receiving home health care services.
• Provides necessary support to team members involved in client care.    
• Demonstrates ability to travel to/from patient’s homes, maintaining timely and accurate reports and records.
• Demonstrates respect for patient confidentiality, goals, property and lifestyle.
• Evaluates the client’s psychological status as it relates to the client’s illness and environment.
• Assists the physician and other team embers in understanding the significant social and emotional factors related to the health problems of individual clients.
• Prepares clinical and progress notes in timely, accurate manner.
• Participates in case conferences with the health care team.
• Works with the family and/or significant others.
• Utilizes appropriate community resources.
• Participates in discharge planning and inservice programs.
• Acts as a consultant to other department personnel.
• Identifies client and family needs for other home health services and refers for same when appropriate.
• Delegates to/supervises social worker assistants.
• Coordinates a treatment and/or supervisory caseload utilizing grouping of visits per locale, time prioritization, and demonstrating productivity.
• Ensures safety, infection control, and maintenance of equipment in office area and in the patients’ home.
• Participates in the agency’s planning for social service programs and policy formulation.
• Assists with Performance Improvement activities within the departments/hospital to improve patient/staff and quality of patient care, etc.

• Master’s degree from a school of social work accredited by the council on social work education.
• One year of social work experience in a health care setting including age-related competence for pediatric, adolescent, adult, and geriatric patients as indicated. 
• Current licensure in Massachusetts.
• Knowledge of growth and development across the life span.
• Certification in BCLS of Adult.
• Must have valid driver’s license and access to reliable transportation.
• Working knowledge of therapeutic modalities and community resources.
• Must demonstrate good communications, verbal and written skills.
