Dear Ms. Grasso, I just wanted to take a minute to commend you and your staff. My mother, Gilberte H., has been in and out of Port Rehab since November 2018. It does help when I am there and witness the care and compassion extended to her in her time of need. I wish I could think of the names of all the staff there who I feel have not only done their job well, but went over and above, but I will mention the few names that stand out to me. Cara has been especially patient and kind with Mom. Paul makes her smile and laugh, and the one who really prompted me to even think to write this letter is David. I believe he’s an LPN; he administers her medications. He treats her with incredible compassion, gentleness, and kindness and allows her the dignity she deserves. I don’t want to leave out all the Occupational and Physical Therapists who work with Mom. They are all exceptionally professional and wonderful. Social Worker - Full Time for Subacute Unit

Primary Responsibilities:
• Participates in the formation of the total care plan for each resident based on medical, nursing and psycho­social needs of the resident.
• Develops the social service evaluation for the clinical record to include the resident's social history, social and emotional need of the resident, working goals and treatment plan.
• Implements working knowledge of Medicare/Medicaid by­laws.
• Participates in weekly Medicare meetings, works closely with rehab and case management to develop discharge plan for short term resident to ensure successful return to community.
• Provides all appropriate community referrals for short term residents prior to discharge to ensure safe discharge back to community.
• Remain available and provide consultation to residents/families in efforts to solve their questions or concerns.

• Must be Licensed by the Commonwealth of Massachusetts.
• Must have a least a BA in Social Work or other related discipline from an accredited university.
• At least two (2) years experience as a Social Worker required with experience in a Healthcare setting preferred; work experience with the geriatric population a plus.
• Possess working knowledge of State and Federal programs and regulations affecting nursing homes preferred.  Possess general knowledge of community health and social resources preferred.
