The WHH Experience
Upon admission to Whittier Home Healthcare, you are assigned to a clinician who manages your care. This specialist performs a full, comprehensive assessment to determine all of your medical and environmental needs.
Collaborative
Working with you and your primary care physician, your case manager develops a plan for your care and enlists a team of clinicians to address your individual needs. Your case manager will coordinate all of your care needs with your physician and other caregivers to ensure comprehensive, patient-focused care.
