Make a Patient Referral

Admission Guidelines

Physicians/office staff and VNAs must specify that they are looking for an inpatient admission from the doctor’s office or from home when they call our central admission line at (800) 442-1717.

Patient demographics, inclusive of the insurance carrier and policy number(s), along with pertinent clinical information outlined below will need to be faxed to (978) 478-3040.

How Does My Family Member Get Referred to Whittier?

A referral is made by your doctor, case manager, or social worker. Appropriate patients can also be admitted from the Emergency Room or home. If you would like to advocate or make a referral for your loved one’s inpatient admission, we can help coordinate care through our Central Admissions Office at (800) 442-1717, or you can contact your hospital clinical liaison directly.

Referrals and admissions to individual nursing homes and subacute facilities are made by contacting each facility individually. Please visit these Skilled Nursing Facility and Transitional Care Unit pages for contact information.
