Nemasket Healthcare Center announces new inclusion to the Steward Integrated Care Network

Nemasket Healthcare Center, a Five Star Skilled Nursing Facility, in connection with Morton Hospital, a member of the Steward Health Care Network, announces the inclusion of Middleborough based Nemasket Healthcare Center to the Steward Integrated Care Network. The inclusion took place as of January 1, 2016 with the goal of allowing hospital doctors or other primary care providers to communicate closely with the Nemasket Healthcare Center to deliver high-quality care that meets the individual needs and preferences of short or long term rehab patients in the greater Taunton and Middleborough area. 

Nemasket Healthcare Center believes there is a better way to provide care. Hospital and community physicians will enjoy credentialed privileges with Nemasket Healthcare Center allowing for more continuity of care to be provided through the use of comprehensive therapy and 24hr nursing and physician support. “We have always worked closely with Morton Hospital and the people in our community but we couldn’t be happier with this integration with Morton and Steward.” said Greg Messina, the Administrator of Nemasket Healthcare Center. “By working together to provide care that meets the needs of patients in our community, we can improve their quality of life which is our entire focus as a healthcare provider.” 

As a member of Steward Health Care Network, the Nemasket Healthcare Center team of providers will communicate with a patient’s Steward physician(s) to provide integrated and efficient care, sharing information needed to create patient specific wellness programs. Local health care providers will work closely with the facility to create treatment plans to manage patient’s overall health, focusing on recovery and long term wellness, while shortening the length of time patients would need be away from home.
