My nana was placed here over the holiday season. I was reluctant to place her, however, after such a wonderful experience with the transitional rehab unit, we decided it was our best bet to keep her safe. The nurses and staff at the transitional rehab unit are second to none! What an amazing group of people. So kind and helpful, I can never thank them enough. The activities coordinator was also so kind to my nana. She was transitioned into the long term care at green leaf and all of the staff there has been so patient and kind to us. I must say her case manager Nancy Adams is an angel, and has worked with us through any issues or questions we have had. She is absolutely the most caring helpful person I have come across. I don’t think this transition would have gone as well as it did if not for her. You can tell she really cares about the people who call Hannah Duston their home and truly wants them to be happy. Thank you to the whole staff for making my nana feel at home. I appreciate all of your support.
